Employer coverage and Medicare: ask the right questions

An insurance card does not tell you whether Medicare should pay first. A short conversation with the benefits administrator can prevent a costly assumption.

Ask who pays first

For people eligible for Medicare because they are 65 or older, employer size often affects which coverage pays first. Different rules can apply for disability, ESRD and multi-employer plans. Confirm the rule for your own coverage rather than relying on a coworker’s experience.

Do not treat COBRA like current employment

COBRA can continue employer-plan coverage after employment ends, but it does not extend the employment-based Part B enrollment deadline. Retiree coverage also needs a separate review. Ask whether the plan requires enrollment in both Part A and Part B.

Keep a written record

  • Name of the employer and the plan administrator.
  • Whether coverage is based on current employment.
  • Which plan pays first after Medicare eligibility.
  • Last day of active employment and last day of coverage.
  • Whether prescription coverage is creditable.
